Stallion Service Report

We offer two ways to submit WPCSA forms. You may either print a hard copy and snail mail by choosing the “print and mail form” button or use the online form below to submit your form. If payment is required the online form allows for payment with a credit card. If you choose this option, please make sure you receive a confirmation of receipt email.

Please fill in all of the above fields. For each additional mare simply click the + symbol to the right as many times as are needed to fully complete your Stallion Service Report. ALL FIELDS MUST BE FILLED IN FOR EACH MARE WITHOUT EXCEPTION! If you wish to waive the need for a Breeder's Certificate type "Yes" into that column for each mare. "No" or leaving the column blank requires the mare owner to provide an official Breeder Certificate to register the resulting foal. By typing "Yes" you are authorizing the resulting foal from that mare to be registered with WPCSA without further paperwork from you as the stallion owner. IS = Inseminated with Fresh Semen, NS = Natural Service (Live Cover), TS = Transported Fresh Cooled Semen, FS = Frozen Semen (Transported or on farm use). To your Knowledge, did an Embryo Transfer (ET) take place? - Yes or No

Please upload a broadside photo of the stallion with all markings clearly visible (including face markings) if this is the first SSR being filed for the stallion. Please remember that you MUST also mail the Original Certificate of Registration back to the National office if the stallion has not already been recorded with a Stud Date.

If your stallion does not already have a listed Stud Date on his papers you must file a One Time Fee of $75 with his initial SSR along with the photo requested above.

If this is the first ever SSR for this stallion this fee is waved and should be left blank.
Additional Fee for Late Submission
It is month late upon the 1st of each month. For example: on January 1st the SSR is 1 month late. Corrections constitute a late submission.

You may add the ordering of a DNA Kit with the submission of this registration form. ALL stallions are REQUIRED to be DNA typed to be stud registered. If your stallion does not already have DNA on file with WPCSA you MUST order a DNA kit.
