We offer two ways to submit your WPCSA Registration Forms. You may either print a hard copy and snail mail by choosing the print buttons to the right or use the on-line form below. Please print both pages and remember to include the required photos with your print and mail form if submitting via snail mail. Scroll down to view fees and description of required photographs.
To submit electronically you will need to print out the markings diagram, draw the markings in, and then attach that to the electronic form. The two easiest ways to get the form back onto your computer is to either scan the form or take a photograph of the form with your phone and e-mail it to yourself. Save the diagram on your computer and then attach it in the electronic form below. It is preferable for the form to be saved as a pdf, but jpeg is acceptable as well. If you choose to submit electronically, please make sure you receive a confirmation of receipt email.
Remember to double check the e-mail for accuracy in the form below. A copy of the registration application will be e-mailed to you for your records. A payment receipt will also be e-mailed to you. If you fail to fill in all the required fields the form will not submit and will mark the errors in red for your correction. If you do not receive a copy of the form or a payment receipt contact the office immediately.
