We offer two ways to submit WPCSA forms. You may either print a hard copy and snail mail by choosing the “print and mail form” button or use the online form below to submit your form. If payment is required the online form allows for payment with a credit card. If you choose this option, please make sure you receive a confirmation of receipt email.

I hereby acknowledge that I have read and agree to be bound and abide by all Welsh Pony and Cob Society of America guidelines for artificial insemination. I understand that noncompliance with these guidelines will be reviewed by the Board of Directors and any resultant foals may not be eligible for registration. It is also acknowledged that all artificial insemination breedings are my responsibility, regardless of who performs the work. It is also acknowledged that the Welsh Pony & Cob Society of America is held harmless and free of liability in all cases of artificial insemination, any representation, agreements or guarantees between any and all parties involved. I agree to pay the $125 (non-refundable) application fee and hereby request an Artificial Insemination Permit. I understand that the annual Stallion Service Report must indicate any mares artificially inseminated. I hereby certify that as the stallion owner or lessee I am an active member of the Welsh Pony and Cob Society of America. I agree to the guidelines of the Welsh Pony and Cob Society of America and I understand I will lose the right of registering any foal inseminated artificially should the authorized permit be removed for any infraction of the regulations governing the use of Artificial Insemination.

You may add the ordering of a DNA Kit with the submission of this form. All stallions MUST be DNA typed to be stud registered. If your stallion does not already have DNA on file with WPCSA you must add a DNA kit to this form.

This Penalty is only for stallions that report AI on a stallion Service Report without an AI Permit.

If your stallion has not already submitted a Stallion Service Report and been given a Stud Date on his papers you must do that now with the issuing of the AI Permit. Remember that you must also submit his Original Certificate of Registration to the National Office via Mail at this time as well.
Max. file size: 512 MB.
If you are subject to the above One Time Fee you must also submit a One Time Photo of the Stallion. Photo should be taken broadside with all markings (including face markings) clearly showing. Again, remember that you must send the Original Certificate of Registration to the National Office.
